We report a case of dynamic foraminal stenosis caused by comminuted fracture of the lumbar vertebral endplate in a patient with osteoporosis. The patient complained of severe back and right leg pain, which was relieved in the supine position and was markedly aggravated during walking. Magnetic resonance imaging showed an acute compression fracture of the L4 vertebral body. The patient underwent percutaneous vertebroplasty at the L4 vertebral body, which did not improve leg pain at all. A second open surgery revealed dynamic foraminal stenosis due to a comminuted endplate facture of the L4 vertebral body. After the second surgery, the patient showed the marked improvement in her leg pain.
